Original and photocopy of Certificate of Good Moral Character
Original and photocopy of PSA birth certificate
3 copies of recent 2×2 ID picture
2 copies of recent 1×1 ID picture
2 long brown envelopes
1 long white folder with plastic cover
For Transferees/2nd Coursers:
Original and photocopy of Honorable Dismissal
Original and photocopy of Transcript of Record for evaluation
Original and photocopy of Certificate of Good Moral Character
Original and photocopy of PSA birth certificate
